    Example:

    The National Center for Interprofessional Practice and Education has a conflict of interest policy that requires disclosure of financial relationships with commercial interests. All presenters of the learning activities at this event must disclose any financial relationships to the National Center and the learners of the activity.

    Please submit completed disclosure forms, biosketches, and CVs for all individuals planning to present the abstract if accepted.
